Employee Training Requirements
All Employees possess professional safety training and certifications including, but not limited to, the following list below. Required training is assigned and/or verified per the findings of the completed Pre-Task Safety Plan and Hazard Control Measures assessment. Pertinent employee training requirements are implemented in the complete Written Hazard Assessment during the project planning phase.

Safety begins at the top level of any operation. StopLoss Specialists ownership and management has cultivated a “culture of safety” as one of the pillars of our core value system. Our corporate manual is custom written to not only be informative, but also to be applicable in the field and enforce accountability to internal management, employees, and subcontractors. All employees and subcontractors are indoctrinated and held accountable to the standard of the StopLoss Specialists approach to safety. All employees and subcontractors are empowered to stop work flow processes at any time that they feel safety is an issue.
StopLoss Specialists has an active “Safety Committee Program”. The committee meets monthly, performing essential monitoring, educational, investigative, and evaluative tasks.
Hazard Communication Program
StopLoss Specialists has compiled and implemented a thorough hazard communication program complete with specific directives for management, training checklists, and training acknowledgement forms for employees and subcontractors.

A. Site Specific Safety Plan / Evaluation
Prior to commencement of work, a StopLoss Specialists safety professional visits site, reviews scope of tasks to be performed, equipment to be used, and work area hazardous substances/chemicals that may be present from beginning to completion of project. He/She then formulates a detailed, comprehensive site specific safety plan including, but not limited to, the following information:
Full Site Background, Aerial Site Views, and Closest Emergency Medical Locations – Onsite and Offsite Emergency Contacts – Emergency Evacuation Plan/Escape Procedures and Route/ Employee Accountability – Major Tasks Identification and Hazard Evaluation Forms – Pre Task Safety Plan – Hazard Control Measures – Written Hazard Assessment per the Work Activity Sequence – Work Activity Equipment Lists w/ Inspection and Training Requirements – Personal Protective Equipment Plan and Training meeting Record – Task Respiratory Plan – Hearing Protection Needs – Site Specific Employee/Subcontractor Training Verification – Pre Emergency Planning – Work Area Hazardous Substance List including Chemical/Serial#/Vendor/MSDS Info –
PROJECT SAFETY CONTROLS: Implemented throughout the project.
B. Site Specific Safety Training & Programs
StopLoss Specialists management is required to keep an ongoing jobsite observation record of safety for both employees and subcontractors. Daily safety “tailgate” meetings are administered and documented, covering both general safety guidelines and specific safety guidelines depending on the scheduled tasks for that day. Other forms of documentation are used throughout, including but not limited to, the following:
Safe Work Observation Forms – Construction Inspection Forms – Equipment Inspection Forms- Ongoing Health and Safety Assessment Checklists covering the following: Hand and Power Tools, Earth Moving Equipment, Concrete and Masonry, Aerial Lifts and Ladders – Daily PPE Safety Inspection and Enforcement following OSHA Inspection Procedures – Ongoing Site Plan Evaluation and Updates as needed –
D. Incident Reporting & Safety Documentation
As listed above, all aspects of the StopLoss Specialists safety program are documented in writing beginning with our corporate safety manual. All training, both in-house and site specific, all evaluation & assessments, and any disciplinary or corrective action taken is written to create a permanent safety record for StopLoss Specialists In the event of a safety incident, including a near miss or injury, the incident is thoroughly documented and investigated via Supervisor, Employee, and Third Party Witness Statements. The Supervisor’s Investigation Form is then used to produce a sequence of all pertinent facts in a time table format so that a Root Cause Analysis Flowchart can be applied. The Root Cause Analysis Flowchart is then used to adjust safety protocol if the cause is applicable to correction, and is used as a teaching tool to prevent re-occurrence if the cause is applicable as a teaching preventative tool.
E. Sub-Contractor and Employee Safety Requirements
StopLoss Specialists requires its Sub-Contractors and Employees to actively participate in the project Health and Safety Program equally and attend all required safety meetings. Sub-Contractors must also have their own internal safety policy that meets OSHA standards pertinent to their field of work, as well as all meet all applicable insurance coverages required. Sub-Contractors and Employees are monitored by StopLoss Specialists management via the Project Safety Control documentation. If non-compliance or unsafe conditions are observed, Sub-Contractors and Employees are sited in writing for the offense, and affected work is stopped until adequate corrective measures are implemented. If repeat non-compliance is observed, the Sub-Contractor or Employee is removed from the project.
